If unable to complete the referral online, referral form is available for download.
Completing a Referral form should take approximately 10-15 minutes.
Referral should be completed together with the family whenever possible.
Prior to submitting a referral, you should ensure that the family have provided consent for engagement with
the KEYS Network.
The family should be provided with a copy of the completed Family Referral Form, if requested.
The KEYS Network is not a crisis response agency. However, if you have a family with an urgent service need
please contact the KEYS Helpdesk prior to completing the referral on (02) 8811 7145
